Everyone makes insurance companies and pharma out to be the bad guys, but the main drivers of our higher healthcare costs are hospitals and doctors. The cost of hospitalization is like 10X what it is in other countries, and doctors make 2-3X or more of what they get in other countries (and still complain about student loand and being underpaid).

The purchaser and the provider need to be 'the market', and the government, and ambulance chasers, and insurers need to get out the way. Insurance should be for stop gap coverage and catastrophic events.
