I didn't get to see the game live, but that was the first thing I noticed when I saw the replay. There was no doubt he was out of the box, but like mantooth said it is rarely if ever called. It happens so quickly and it would be nearly impossible for the home plate umpire to see "through" the catcher to where Cabrera's back foot was when he made contact with the ball.
