Indiana -- Last team do it, 1976
UCLA -- I know they did it twice during their 88-game win streak, I wouldn't be surprised if they did it another time as well
San Francisco -- I believe they were the first team to do it -

fictionEhh. I don't know other than just listing off big programs. I think NC St. technically finished a season undefeated, since they were on probation and couldn't accept an NCAA bid.

UK 25-0 in 1953/53 (turned down bid because 3 players were ruled ineligible for the NCAA tournament) -
