Internet Guru's: Help needed finding best online Test/Quiz application

AppleWe're looking for a good online test/quiz application at work for online educational activities. Basically, we have educational videos offered on our website that people can view and after taking the online test, can be awarded continuing education credit.
We've gone to a new CMS and we just aren't able to get the results we want with online tests/quizzes so I'm starting to research it.
I am hoping maybe someone on the OC has experience and could give me suggestions. -
AppleThings we are looking to be able to do:
1. Create 10-12 individual tests/quizzes per year where part of content remains constant on each test while the first 3-5 questions change on each test.
2. Constant content is a rating system (excellent to poor) for the educational activity (Objectives met, quality of presentation etc.)
3. Constant content area must also have at least 3 boxes where responder can type in an answer or make comment
4. The 3-5 questions will be specific to the educational activity and can be True/False or multiple choice.
5. Each individual test must be accessible for a minimum of 2 years
6. Tests need to be auto-graded and results emailed to test taker and to 2 of our internal people -

Answer. I buy beer tonight until you all can't walk! ( deal Breaker. don't hire this guy. he'll get your job eventUALLY)
